This survey is voluntary and confidential; we will not ask for your name or any details that could be used to identify you. The anonymous data collected from this questionnaire may be presented on the ARFNZ website, in the ARFNZ Better Breathing magazine, included in ARFNZ social media posts or other ARFNZ-generated resources. The survey will close on 8 April 2023.
