This is a brochure containing a chronic obstructive pulmonary disease (COPD) Action Plan for doctors to complete together with their patients. It includes actions to take for different levels of symptoms for COPD – from well, worsening, severe to emergency.
These plans are self-management action plans for adults to be completed by doctors together with their patients with asthma. Also available in English, Samoanas and Te Reo Māori.
